"No-Show" Policy

If you make an appointment but are unable to keep it, please let us know at least 24 hours before your appointment time if possible. That will allow other patients to have the opportunity to schedule during your canceled time slot. You can call or send an email to let us know that you need to cancel or reschedule. Patients who "no-show" once (i.e., miss an appointment without notifying us in advance) may be blocked from scheduling online and any future appointments already scheduled may be canceled. Also, we may no longer accept any appointments from patients who no-show twice. Thank you for keeping us informed and, especially, being considerate of other patients.

Regular Acupuncture Treatment (without insurance)

Initial visit: $120, Follow-up visit: $95
3 treatment package: $265 ($88/tx, offer valid from 2nd visit)
5 treatment package: $415 ($83/tx, offer valid from 2nd visit)
10 treatment package: $770 ($77/tx, offer valid from 2nd visit)

Fees above apply only if paid at Time of Service or before in cash or by check, debit, or credit card.

These fees do not apply if a patient is using insurance (see Insurance Plans Accepted below and check with your insurance plan).

Packages expire one year from the date of purchase. Unused treatments will not be refunded after one year.

Discounts for Seniors 60+ & full-time Students with ID (without insurance)

Initial visit: $110, Follow-up visit: $85
3 treatment package: $240 ($80/tx, offer valid from 2nd visit)
5 treatment package: $375 ($75/tx, offer valid from 2nd visit)
10 treatment package: $700 ($70/tx, offer valid from 2nd visit)

No discounts apply if a patient is using insurance. Only one discount applies for each treatment.

Cupping (cupping is not covered by insurance)

Cupping only: $70
Cupping added after completing insurance-covered Acupuncture treatment: $25

Insurance Plans Accepted
I am an in-network provider for the insurance companies listed below. Please note that not all plans administered by these companies cover acupuncture. Also, coverage will usually require copays or coinsurance payments and be subject to deductibles. If insurance coverage is important to you, please be sure to call your insurance company and ask them to explain your Acupuncture eligibility and benefits before making an appointment with me. Also, please note that I cannot accept Medicare or Medicare Advantage plans. I do offer a Senior 60+ discount, but if you plan to use Medicare or Medicare Advantage please do not schedule an appointment with me. Thank you.

I can accept Ohio Medicaid offered by the plans listed below for a limited number of conditions including low back pain, migraines, cervical (neck) pain, osteoarthritis of the hip, oseoarthritis of the knee, and nausea or vomiting related to pregnancy or chemotherapy. Medicaid patients pay no copays, coinsurance, or deductibles and can usually receive up to 30 acupuncture treatments per year without a referral or prior authorization. Please note that I can only accept Medicaid if it is your primary insurance.

Ohio Bureau of Workers' Compensation

I can accept Ohio BWC patients with completed C-9 forms that have been approved and signed by a managed care organization (MCO). Please contact your physician of record (POR) or BWC case manager for more information about how to request acupuncture services. Health Savings Account (HSA)/Health Reimbursement Account (HRA)/Flexible Spending Account (FSA)
Acupuncture is a qualified medical expense for all HSAs and most HRAs and FSAs. I can accept health account debit cards and checks, or upon request, I can provide a detailed receipt for your records so that you can be reimbursed from your HSA/HRA/FSA for the cost of your treatment.

Other Important Information Concerning Insurance:

Patients are responsible for checking and understanding their insurance plan's Acupuncture eligibility and benefits. Any claims denied by insurance complanies must be paid in full by the patient within 30 days.
The following services are usually not covered by insurance plans and must be paid at Time of Service: Cupping, Fertility Treatments, Acupuncture Facial Rejuvenation, Microneedling & Nanoneedling, Oriental Medicine Consultations & Chinese Herbal Medicine Prescriptions.
