These fees do not apply if patient's treatment is covered by insurance (see Insurance Plans Accepted below).

Packages expire one year after the 1st treatment. Unused treatments will not be refunded.


Discounts for Seniors 60+, Students, & Discount Programs (without insurance)

  • Initial visit: $95, Follow-up visit: $75 
  • 3 treatment package: $210 ($70/tx, offer valid from 2nd visit)
  • 5 treatment package: $325 ($65/tx, offer valid from 2nd visit) 
  • 10 treatment package: $600 ($60/tx, offer valid from 2nd visit)

Fees above apply only if paid at Time of Service or before in cash or by check, debit, or credit card.​ 

No discounts apply if patient's treatment is covered by insurance. Only one discount applies for each treatment. 
Packages expire one year after the 1st treatment. Unused treatments will not be refunded.


Cupping

  • I no longer accept new appointments for Cupping only. Cupping may still be used in combination with needling during Acupuncture treatments that are not covered by insurance.
  • Since Cupping treatments are not covered by any of the insurance plans I accept, I am not able to include Cupping as part of insurance-covered Acupuncture treatments.
  • Cupping added after completing insurance-covered Acupuncture treatment: $20

I am an in-network provider for the insurance companies listed below. Please note that not all plans administered by these companies cover acupuncture. Also, coverage will usually require copays or coinsurance payments and be subject to deductibles. If insurance coverage is important to you, please be sure to call your insurance company and ask them to explain your acupuncture eligibility and benefits before making an appointment with me. Also, please note that I cannot accept Medicare or Medicare Advantage plans (except for the MediGold plan listed below).

Ohio Medicaid

Ohio Medicaid covers acupuncture for low back pain and migraines. Patients pay no copays, coinsurance, or deductibles and can usually receive up to 30 acupuncture treatments per year without a referral or prior authorization. I can only accept Medicaid if it is your primary insurance.

MediGold (Mount Carmel Health Plan) Medicare Advantage Plans

Most MediGold plans in Ohio cover either 6 or 12 acupuncture visits per year with a $20 copay and no deductible. Please be sure to call the customer service number on the back of your card to verify that your specific plan includes acupuncture benifits and how many visits are allowed before making an appointment with me.

Ohio Bureau of Workers' Compensation

  • I can accept Ohio BWC patients with completed C-9 forms that have been approved and signed by a managed care organization (MCO). Please contact your physician of record (POR) or BWC case manager for more information about how to request acupuncture services. C-9 forms can be faxed to 614-305-6040, Attention: Mira Choi Tyson, L.Ac.

​I am a certified provider for the following programs that are not insurance but offer discounts for acupuncture services. Please check with your insurance company to see if they offer one of these value-added benefits. To qualify for a discount, you will need to mention the name of the program you are eligible for and present either your insurance ID card or your discount program membership card before your first treatment.

ChooseHealthy Affinity Discount Program 

  • Participating health plans include: Aetna, Anthem, Cigna, and others.​

WholeHealth Living Discount Program

  • Participating health plans include: Humana, MediGold (Mount Carmel Health Plan), and others.

Health Savings Account (HSA)/Health Reimbursement Account (HRA)/Flexible Spending Account (FSA)
Acupuncture is a qualified medical expense for all HSAs and most HRAs and FSAs. I can accept health account debit cards, or upon request, I can provide a detailed receipt for your records so that you can be reimbursed from your HSA/HRA/FSA for the cost of your treatment.

"No-Show" Policy

​If you make an appointment but are unable to keep it, please let us know as far in advance as possible. That will allow other patients to have the opportunity to schedule during your canceled time slot. You can call, text, or send an email to let us know that you need to cancel or reschedule. Patients who "no-show" once (i.e., miss an appointment without notifying us in advance) will be blocked from scheduling online and any future appointments already scheduled will be canceled. Also, we will no longer accept any appointments from patients who "no-show" a total of three times. Thank you for keeping us informed and, especially, being considerate of other patients.
